Better Care with On-Site Hemodialysis
Whether you have recently been diagnosed with kidney disease or have already been receiving dialysis therapy, it's critical to understand all of your options for care. Receiving your hemodialysis treatments on-site at Transitions Healthcare North Huntingdon is an option.
On-site home hemodialysis provides the opportunity to tailor treatments specifically to you and your medical needs. Advantages of on-site home hemodialysis may include:
- Individualized care to help you maintain meal, medication and treatment schedules
- No more transportation
- More time for rehabilitation and socialization
- Access to flexible treatment options including more frequent hemodialysis
Reclaim Your Day with More Frequent On-Site Home Hemodialysis
Unlike conventional hemodialysis, which is typically performed 3 days per week, more frequent home hemodialysis better mimics how healthy kidneys function. Patients receive shorter treatments more often, and, compared to conventional hemodialysis, there is less time for fluid and toxins to build up between treatments. In addition, many patients who perform more frequent therapy experience:
- Less cardiac-related hospitalizations
- Improved energy and vitality
- Improved appetite
- Reduction in the number of medications taken
- Reduction in the stress placed on the heart

Take a look at the difference we make…
ProForm Recumbent Bike is a low impact exercise bike with customizable settings, used to improve cardiovascular health, alleviate joint pain and build strength and endurance.
Nu Step Cross Trainer delivers a low-impact, total body work out that can benefit users of virtually any fitness level or functional ability.
It is indicated for use with patients rehabilitating from joint replacements as well as patients with chronic health conditions coupled with cognitive decline in order to build strength and cardiovascular endurance to enhance mobility. It is often utilized for orthopedic, neurological, cardiovascular and pulmonary diagnoses.
Diagnoses which may benefit from the use include: CVAs, Cardiovascular Disease, Multiple Sclerosis, Parkinson’s Disease, Diabetes, Osteoarthritis, and Osteoporosis.
Electrical Muscle Stimulation (ESTIM) generates electric impulses that stimulate muscles to contract. This strengthens and repairs tissue and muscles that have been shortened, weakened or atrophied due to injury or disease. ESTEM can slow the progression of neuromuscular disorders and improve motor control.
Stroke Recovery
There’s still so much we don’t know about how the brain compensates for the damage caused by stroke. In some cases, the brain cells may be only temporarily damaged, and may resume functioning over time. In other cases, the brain can reorganize its own functioning. Every once in a while, a region of the brain “takes over” for a region damaged by the stroke. Stroke survivors sometimes experience remarkable and unanticipated recoveries that can’t be explained.
Rehabilitation may mean, in some cases, re-learning basic skills such as bathing, eating, dressing and walking. Our nursing and rehabilitation teams work together with you and your family, to achieve maximum recovery and independence.
Prosthetic and Orthotic Care
A prosthesis is designed to restore and enhance mobility and independence, but learning to adapt and maximize its use can sometimes be very challenging. Our therapists work together with you and your Orthopedic physician to develop a plan and schedule for training and increasing activity. Care and monitoring of a residual limb is an important part of the healing and recovery process. Our therapists and clinical staff are available 7 days a week to help.
Mobility plays a key role in independence. Abnormal motion and gait can sometimes require specially-made devices such as orthotics. They may be used to treat foot pain caused by abnormal motion or to treat injuries. Once a Podiatrist has conducted a complete evaluation of your feet, ankles, and legs, orthotics are manufactured to accommodate your unique foot structure and pathology. Our therapists work closing with your Podiatrist to ensure proper fitting and provide the training you need to get back on your feet!
Pulmonary Care
Our Pulmonary Program seeks to provide the best practice guidelines for treating Chronic Lung Disease, in the post-acute setting, in order to help patients with moderate to severe breathing problems overcome their physical limitations. Using a comprehensive pulmonary evaluation conducted by our respiratory therapists and physicians, we develop individualized care plans of supervised physical conditioning, exercise, dietary and educational counseling.
What is Pulmonary Rehabilitation?
Pulmonary Rehabilitation is an extensive program that encourages improved quality of life for people who suffer from chronic breathing problems such as COPD, sarcoidosis, and cystic fibrosis. Our pulmonary Rehab teams include board certified physicians, respiratory therapists, nurses, physical therapists, occupational therapists, and social workers.
Our Pulmonary team specializes in treating patients with a tracheostomy, Bi-Pap, C-Pap, V-Pap, or A-Pap needs.
