It is indicated for use with patients rehabilitating from joint replacements ranging to patients with chronic health conditions coupled with cognitive decline in order to build strength and cardiovascular endurance to enhance mobility. Stroke Recovery
There’s still so much we don’t know about how the brain compensates for the damage caused by stroke. In some cases, the brain cells may be only temporarily damaged, and may resume functioning over time. In other cases, the brain can reorganize its own functioning. Every once in a while, a region of the brain “takes over” for a region damaged by the stroke. Stroke survivors sometimes experience remarkable and unanticipated recoveries that can’t be explained.
Rehabilitation may mean, in some cases, re-learning basic skills such as bathing, eating, dressing and walking. Our nursing and rehabilitation teams work together with you and your family, to achieve maximum recovery and independence.
Pulmonary Care
Our Pulmonary Program seeks to provide the best practice guidelines for treating Chronic Lung Disease, in the post-acute setting, in order to help patients with moderate to severe breathing problems overcome their physical limitations. Using a comprehensive pulmonary evaluation conducted by our therapists and physicians, we develop individualized care plans of supervised physical conditioning, exercise, dietary and educational counseling.
What is Pulmonary Rehabilitation?
Pulmonary Rehabilitation is an extensive program that encourages improved quality of life for people who suffer from chronic breathing problems such as COPD, sarcoidosis, and cystic fibrosis. Our pulmonary Rehab teams include board certified physicians, nurses, physical therapists, occupational therapists, and social workers.

Congestive Heart Failure Program
CHF is a condition in which the heart can no longer pump enough blood to the rest of the body. This condition may affect the right side, left side, or both sides of the heart and can significantly impact quality of life.
Our cardiologists, physicians, nurse practitioners, and dieticians work together with our therapist to develop a care plan that can improve heart function, lesson symptoms, allowing you to lead a more active life.
Behavioral Health/Dementia
Autumn Grove Care Center offers a truly one of a kind, dementia/behavioral care center. All caregivers have advanced training and understand the unique needs of our residents. Staff are trained to provide assistance with activities of daily living, such as eating, bathing, dressing and walking, as well as how to handle the specific needs of those with Alzheimer’s and other dementias. This training includes understanding how the disease manifests, and knowing why dementia patients may exhibit certain behaviors.
Since individuals with dementia may easily become stressed and confused, a special emphasis on creating a relaxing environment is very important. At Autumn Grove Care Center, individual attention is given to each resident by staff whom they’ve grown to love and trust.
Activities are structured for all cognitive levels so that every resident feels a sense of purpose, dignity and value.
