Celebrating Lauryn Hammond: 2024 Stories of Caring Honoree
PHCA ‘All In’ 2024 Convention & Expo
At Transitions Healthcare Shook Home, we have the privilege of working alongside some of the most compassionate and dedicated healthcare professionals. One of those remarkable individuals, Lauryn Hammond, a Certified Nursing Assistant (CNA), was recently honored by the Pennsylvania Health Care Association (PHCA) at their annual Convention and Expo for her exceptional service and commitment to caregiving.
On September 24th, Lauryn was recognized as one of the PHCA’s Stories of Caring honorees, a prestigious award presented to caregivers who go above and beyond their daily duties to provide extraordinary care to their residents and support to their team members. This annual award is designed to highlight those who embody the spirit of caregiving, and Lauryn truly exemplifies what it means to serve with heart and dedication.
The PHCA Stories of Caring Award is a significant honor, with recipients nominated by their peers who share compelling stories about the incredible difference these caregivers make in their communities. This year, the PHCA celebrated caregivers from long-term care communities across Pennsylvania, including those working in nursing homes, assisted living facilities, and personal care homes.
Lauryn’s dedication to her residents and team members earned her this well-deserved recognition. During the convention, she, along with other honorees, was showcased in a personal video that highlighted her journey, her passion for caregiving, and the impact she has made at Transitions Healthcare Shook Home. It was a powerful moment that allowed all attendees to witness the heart behind Lauryn’s work.
We could not be prouder of Lauryn’s achievement. She is a shining example of what makes Transitions Healthcare Shook Home a special place. Lauryn’s kindness, patience, and dedication to her residents inspire her peers and improve the lives of everyone she touches.
The PHCA Stories of Caring Awards are presented in partnership with the Ceca Foundation, an organization whose mission is to improve the human experience in healthcare communities by honoring the work of exceptional caregivers like Lauryn. Together, they shine a light on individuals who are making a profound difference, often in ways that go unnoticed.
Congratulations to Lauryn on this incredible achievement! We are beyond proud of her efforts and the compassionate care she brings to Transitions Healthcare Shook Home every day.
Thank you, Lauryn, for all you do—you are truly one of a kind!
